Yes, there is the phenomenon of 'processed' meat here in the US. I'm not sure how prevalent it is in the UK, but essentially, bits of meat are ground up together and reformed into patties, chicken 'nuggets' and things like that. It's pretty gross.
A few places actually do use 'whole cuts' of meat - which means if you break that chicken nugget open, you see an actual chunk of solid meat with the grain of the muscle visible, not processed mush with fillers and crap added to it.
Hardees and Wendys tend to be two of the better 'fast food' places IMO. Hardees fries are actual cuts of potato, for instance, instead of reformed potato mush or whatever the hell most fries are made from. Better still, some places oven bake the fries instead of deep frying them.
