I'm going in for a Goddamn Colonoscopy and they send me a book to fill out, then they say register online, then at the first meeting you fill out a 6 page thing, then when you go in they give you another one to fill out that covers all the other ones.
I went on-line and registered skipping every single question and at the end I said in the comments. "Fill out your own form ya lazy pricks, I've only filled this out 3 goddamn times and I'm not your fucking data entry bitch. Just shove the fucking camera up my diarrhea, hemorrhoid infested ass and give me a coffee, then send me on my fucking bowlegged way."
