I don't get the neck fucking at all

Back when Kennedy was assassinated, the discovered LBJ in the morgue hospital room. He was fucking Kennedy in the neck. When asked why he replied "To fool the Warren Commission into thinking it's an exit wound".

True story. That's where the term came from. Tony Hendra first coined it as the above joke back in the National Lampoon days. I found it to be very unnerving when I first co-opted it because it seems shocking, certainly more powerful than "fuck you in the face" or "fuck you in the throat".
