The ER closest to me is fast, usually you're seen within 10 mins. If the doctor decides that it isn't an emergency then you have to pay $150 co-pay whether you have insurance or not before they'll treat you. So for example if you go in there at 3 in the morning because you have the flu and your chest is on fire, you'll be in and out of there with a prescription for cough syrup with codeine in 20 minutes as long as you pay the $150 first. If you don't have the $150 they'll give you a list of the closest urgent cares to go to.
